A report that Iran's supreme leader, Mojtaba Khamenei, has fallen unconscious is stoking concern over a possible power vacuum as the war continues.

The Times of India reported on July 7 that Khamenei was unconscious and receiving emergency treatment in Qom, Iran. It cited a diplomatic document based on assessments by US and Israeli intelligence agencies.

According to the report, Khamenei is in "severe condition" and unable to take part in the Iranian regime's decision-making process. It also said this was the first known instance in which his whereabouts were disclosed. Intelligence agencies had previously identified his location but had not made it public.

Iran's government had earlier acknowledged that Ali Khamenei was killed in an airstrike on Feb. 28 and that Mojtaba Khamenei was also wounded. Even so, authorities continue to claim he remains in charge of running the country. Since the war began, no direct public appearance through video or audio has been confirmed. Only written statements carried by state media and AI-generated videos have been released.

The situation is deepening fears of a power vacuum. Iran's supreme leader holds final authority over political and religious affairs, and any inability to make decisions could affect the functioning of the system as a whole. Some have also raised the possibility that the Islamic Revolutionary Guard Corps, or IRGC, is exercising de facto power.
